Spring Sports Physicals March 4th 5-8pm

Free spring sports physicals (boys lacrosse, girls lacrosse, and softball) will be held on Wednesday, March 4 from 5:00-8:00 pm at the Coordinated Health Emrick Boulevard location. They will offer us a second date later in March for the players that can't make this date. Please click here to complete the Youth Sports Physical paperwork. Complete the entire packet except for page 5, which will be completed by the physician that evening. Click here for directions to the Emrick Blvd location.
